Description

As soon as the 4.5 DTD is installed on Quantum, and LFS does the conversion, all the XML files will need to be updated to use the 4.5 DTD.

The DTD is installed on quantum and anduin, and the current SVN files in LFS have been converted. I'll convert the BLFS files this evening. Any changes should merge peaceable with anyone's sandbox, as only a couple of lines in the header information will be changing in each file.

Remember to update also stylesheets/blfs-profile.xsl

Updated all the XML files and the one stylesheet to the 4.5 DTD.

Also ensured that the book validates and renders on Anduin.
