Appendix in PDF sections should be "underneath" the heading

When you look at the bookmarks in the PDF verion of the book, some of them have
a plus mark next to them indicating there are sub-parts to the topic(such as
the section called "Book Version"). I think the Appendix should be set up in
the same way. The bookmark "Appendix A. Package descriptions" should have a
plus next to it. Underneath it should be "introduction", "Bash", "Binutils",
etc..
This is just minor issue, but I think it would help organize the book better.

To do this, it will mean that all <sect1> tags change into <sect2> tags. The side-effect is that entire appendix a will be put on one single page. That will look quite bad in HTML. I will see if I can convince htmldoc to make these subsections starting with sect1 instead of sect1.
