for now, I'm only merging appendices a and b and putting the new appendix a in alphabetical order. appendix c will become b, d will (for now, become c). I'll let you know when I'm finished as this is likely to be a rather large CVS change.

a and b merged.

waiting for clearance to merge d into this then (if given) will delete dirs appendixc and appendixd. (old appendix c is now in appendixb).

this is too confusing :-)

Phew. Appendices A, B and D are now all in one (rather large) appendix A. It just needs to be checked now.
