Description
The book outlines in setting up /etc/inittab to use /etc/rc.d/init.d/rc.sysinit, but this script is not included in the text nor in lfs-bootscripts-1.12. I made a workable script from /etc/rc.d/init.d/rc, but it took me awhile to figure out why the boot was failing. After putting the script in place, boot worked great!

No it doesn't. Look at the provided inittab carefully. There is a space in between "rc" and "sysinit". i.e. it's supposed to look like:
si::sysinit:/etc/rc.d/init.d/rc sysinit
This is the script that I used to eliminate the bug.
There is no bug here. I suggest you "man inittab" for all the details. I think you've probably made a typo in your inittab somewhere. This bug is invalid. Marking as such and closing.
Script for missing file.
