Based on PLFS research, need to use no-fix-include patch and eliminate bootstrap, this research needs to be pre-1.0

I don't think it is advisable to replace "make bootstrap" by "make".

the instructions still use "make bootstrap".

Version in book is 3.2.1 to match lfs-cvs.
