Myself, Chris Wesdorp <chriswesdorp@…> and Chris Sheppard <cesheppa- lfs@…> have all reported this in recent history (lfs- dev/lfs-support)

I've tested the standard method for fixing this problem and it resolved the issue. Suggest adding to chapter 5, ASAP. (before 4.0 if at all possible, it's fairly trivial really...)
