/usr/share/info/dir doesn't always get updated properly

Description
Right now my /usr/share/info/dir only has libtool entries (probably from when I reinstalled libtool not too long ago after the entire system was already installed). Other problems have been reported too that the 'dir' file isn't always updated and sometimes just overwritten.
While we can't account for every package in the world, let's at least check all the packages we install with LFS and that all the indexes appear in the 'dir' file as they should be.

I'm going to add a blurb about this problem into the Ch 6 texinfo section. I cannot really find another place that is appropriate. Essentially, it'll give instructions on how to recreate the "dir" file. It will be made clear that the user can run the optional commands at any time, including once in BLFS.
